Case Presentation:

Chief Complaint: 45 yo W F complains about increased blurry vision over the last 6 months (near > distance) and difficulty in dim lighting. She has stopped driving at night 1 month ago. She reports she now has to use a flashlight to unlock her house door at night.

Ocular Hx: possible amblyopia OD

Medical Hx: interstitial cystitis, irritable bowel syndrome, anxiety and PTSD
Medications: pentosan polysulfate 100mg TID last 2 years, clonazepam, atomoxatine, quetiapine

Family Hx: no known retinal hereditary disease or unknown cause of vision loss

Social Hx: on disability 

Clinical Findings: Entering VA 20/25 OD/OS, BCVA 20/20 OD/OS, high near add +2.75 OU, IOP normal, anterior segment unremarkable, DFE findings shown ahead
