The hard exudates (red circles), shown in the OCT images as hyperreflective dots, are composed of lipoprotein and lipid filled macrophages due to the chronic localized leakage of plasma. The exudates are most often located within or near the outer plexiform layer of the retina and sometimes form a circinate ring pattern. The exudates can resolve/absorb in a few months to years. Upon further questioning, the patient revealed uncontrolled blood sugar spikes over the past year.
