The top image reveals another type of hard drusen which disrupts the overlying PIL. These can be referred to as infiltrative hard drusen. The disrupted PIL under the macula corresponds to the reduced VA in both eyes. The bottom image also shows the infiltrative hard drusen, but also reveal that some of the yellow-white spots on the fundus are actually exudates (red circle) due to choroidal neovascularization from the conversion from dry to wet AMD. The exudates can be differentiated from the drusen because they are scattered throughout the outer plexiform layer. The patient was immediately treated with Avastin.
