Case 2: A 7-year-old WM with ADHD presented with BCVA 20/20-2 OD and 20/25- OS. Pupils and externals were within normal limits. Ophthalmoscopy revealed palish discs OU and patient history revealed a mother with light perception VA due to a mt4360 LHON (Leber Hereditary Optic Neuropathy) mutation. LHON is a maternally inherited mitochondrial disorder and all offspring of the mother have the same genetic mutation. Vision loss is typically acute or subacute with an average age of onset of 25. About 50% of males and 10% of females with the genetic predisposition experience vision loss. Prior to vision loss, the discs appear hyperemic, the RNFL appears thickened and the small blood vessels on and around the disc appear corkscrew in shape.
