History/Chief Complaint

  • A 52-year-old female presented with no visual complaints and a family history Of glaucoma. She had been routinely examined over the last decade as a glaucoma suspect and to monitor multiple pigmented lesions in her right eye. Because of the pigmented fundus lesions, 3 colonoscopies have been performed over a period of 10 years and all failed to reveal any polyps or other abnormalities.

Clinical Findings

  • BCVA 20/20 OD 20/20 OS
  • Ta – 20/26
  • 24-2 VF Normal OU
  • GDx: RNFL Thickness Normal OU.

Optos®

  • Revealed two groups of pigmented lesions nasal and superior nasal in the right eye.
  • An area Of white without pressure in the temporal periphery Of the right eye.

OCT

  • Scans through the smaller pigmented lesions revealed no elevation and an attenuated photoreceptor integrity line (PIL) overlying the lesion.
  • Scans through the large pigmented lesion located nasally revealed elevation Of the retina.

Diagnosis

  • Glaucoma suspect
  • Multiple pigmented lesions believed to be CHRPE (“bear tracks”).

Disposition

  • Follow-up examinations twice a year.
