Early stage RPE disruption

History/ Chief Complaint

  • 36-year-old female presented for a routine eye exam with complaints of eyestrain when using the computer. She is on the computer for 6-7 hours daily. She has also had weekly migraines for the last four years. Medications: synthroid and liquigel qhs for dry eyes. Grandmother had dry AMD.

Clinical Findings

  • UCVA 20/20 each eye
  • Retraction: OD +0.50-0.25*110 20/20
    OS +0.75-0.25*157 20/20
  • Amplitude of accommodation: 4D OU
  • Slit lamp exam of the anterior segment showed mild dry eyes and dilated fundus exam was unremarkable.
  • Tonometry (Perkins): OD:17 and OS:16 at 10:30 am
  • Retinal imaging with Annidis RHA: OS showed paramacular RPE disruption.
  • Recommended treatment: Vitalux supplements I tab/day, 2000mg omega 3 fatty acid (EPA/DHA), cold pressed, 2000 IU of vitamin D – concentrated liquid. Four month follow-up with re-imaging and therapy adjustment accordingly.
