Case 5: Optomap® plus with Resmax FAF OD and OS

The AF Bull’s Eye Maculopathy above is quite symmetric and could represent a retinal toxicity, a cone degeneration or an early cone- rod degeneration. Flash ERGs were performed and were normal under all conditions, effectively ruling out a cone or a cone-rod degeneration. The findings are rather typical of plaquenil toxicity RR #2 but the patient has never been on plaquenil.

Could this be Accutane toxicity? An extensive literature review yielded only one such similar case6. If follow-up fails to reveal progression, Accutane toxicity rather than a cone degeneration is the probable diagnosis. Regardless, this case is exemplary of many others in which AF reveals abnormalities not detected with ophthalmoscopy or any traditional type of fundus photography.
